"Do's" and "Don'ts" for Getting Authorizations Accepted

Authorization forms rejected by the healthcare provider can cause delays in retrieving medical records. Check the most common "do's" and "don'ts" to get yours accepted the first time.

DO
Check Spelling. To Do List for Authorizations
Especially patient and provider names.

Check Dates of Service. Be specific or you may get many unwanted pages and fees. Also, make sure to use numeric dating (for example, 1.1.1999 to 10.6.08).

Check Applicant's Signature and Date. Authorizations are often only valid for 60 - 90 days from the date of signature.

Check Disclosures. Indicate that HIV, drugs or psychiatric conditions can be disclosed regardless of the patient's condition.

 Check Legibility.
Make sure there is no hard-to-read handwriting, smears, smudges, etc.

Identify MediConnect as Your Agent. Provide our full name, address and phone number.

 Provide Proper Supporting Documents.
Always include a death certificate and an executor of estate document if the patient is deceased or a letter of testamentary or power of attorney document if the patient is incapacitated.

DON'T

Submit Incomplete Forms. Providers will not release records to MediConnect if the form has blank sections.
